This will significantly reduce your time doing paper-work at our office. You can also login to your patient portal and complete the questionnaires by following the instructions given.
Pre-Visit Forms
These questionnaires are helpful for your provider to accurately diagnose and treat your ENT problem. Please make sure that you complete the applicable forms and submit them online or via email prior to your appointment. If you are a patient having symptoms such as lightheadedness or dizziness, please fill in the Dizziness Questionnaire prior to your appointment.
