If your plan is an HMO or requires a PCP referral, you will need to get that referral sent to us first before we can schedule your appointment. *It is the patient's responsibility to keep up with your referrals expiration date and number of visits remaining. If your referral is expired or you do not have one for your appt you will be considered self pay.
*We do not accept Medicaid. (If you have Medicaid secondary to Medicare or any other Primary Insurance you will be repsonsible for the remaining amount not paid by your Primary insurance at the time of your visit)
